3. You will be REQUIRED to watch the following videos, and sign legal consent forms that you watched these videos, PRIOR to your visit. If you have not watched these videos, you will be denied your appointment until you have watched them.

6. Payment will be required over the phone immediately PRIOR to completing your booking. No exceptions. Forms of Payment include:

   1. Debit/Credit

   2. HSA/FSA

7. After paying for your appointment, you will receive your Welcome Email from OG Chiropractic, as well as your Intake Form, and Informed Consents. These forms MUST be filled out by all who have an appointment, and signed, PRIOR to your visit. If you have not filled out these forms, you will be denied your appointment until you have.
   No exceptions.

8. IMPORTANT: Complete the intake form and informed consents ENTIRELY before exiting your screen otherwise you will have to start over.

WE WILL CONTACT YOU:

• Due to the volume of people requesting this, those that do not follow instructions will not be booked, and will be bumped to the end of the line when all requirements are fulfilled in the email. Please do NOT forget to follow all instructions.

• Jennifer, my wife, will be going through all of the correspondence.

• She will reach you at the phone number provided in your email requesting your appointment after all information requested is provided

• in the order in which it was received

• and won’t finalize an appointment for you, nor reserve a spot for you, without prepayment for all appointments

• responses may take up to 10 business days as there are a LOT of emails, but we will respond to every single one of you

This payment will be non-refundable, but transferrable (to another person known personally by you or to your next scheduled visit) if you:

1. Notify us in writing your need to cancel (7) days or more BEFORE your scheduled appointment

2. Notify us in writing with documentation and proof that you sustained an unavoidable emergency, and it will be left to our personal discretion on a case by case basis
