Here’s How to Schedule:

***Due to the volume of people contacting us to schedule, we are attempting to organize better to make this a more efficient process for us!

***The following are permanent changes to scheduling at one of our OG Chiropractic offices across the US, step by step:

To make an Appt/Contact:

1. Email us at the correct email address for where you want to go:

2. ***FOR NEW PATIENTS: ALL Items MUST be included in the email:

   1. Full name (s) of all who intend on making an appointment

   2. Separate Email addresses of all who intend on making an appointment for intake forms/informed consents

   3. Phone numbers of all who intend on making an appointment

   4. Phone number to be contacted for payment in order to book your appointment(s).

   5. Date(s) requesting, will do our best to accommodate everyone but specific date’s are not guaranteed if the schedule is full.

   9. You will be REQUIRED to watch the following videos, and sign legal consent forms that you watched these videos, PRIOR to your visit.

3. We do NOT treat disease whatsoever. We treat:

4. Most questions can be answered in the videos provided above.

6. Payment will be required over the phone immediately PRIOR to completing your booking. No exceptions. Forms of Payment include:

   1. Debit/Credit

   2. HSA/FSA

7. After paying for your appointment, you will receive your Welcome Email from OG Chiropractic, as well as your Intake Form, and Informed Consents. These forms MUST be filled out by all who have an appointment, and signed, PRIOR to your visit. If you have not filled out these forms, you will be denied your appointment until you have.
   No exceptions.

8. IMPORTANT: Complete the intake form and informed consents ENTIRELY before exiting your screen otherwise you will have to start over.

9. Fee Schedule For All Offices 2025. The fee schedules have been amended to account for increase cost in associated travel expenses for us, cost of opening and maintaining new offices, as well as some added perks to you all, some seen now, and some in the very near future in 2025:

   1. TX

      1. OG Initial Appointment: $300 (required for all new patients)

      2. OG Standard Appointment (follow-up): $100 (for all existing patients regardless of treatment)

      3. Paid follow-ups recommended for New Patients, and existing patients who feel it’s necessary. These appointments reserve available time slots at patient’s convenience with what’s available.

      4. Children 17 and under are FREE **when accompanying a paying adult**

         (***Limited to 2 children / per adult / per visit***.)

      5. LIMITED SPOTS AVAILABLE

   2. AZ / NC / CO

      1. OG Initial Appointment: $495 (required for all new patients)

      2. OG Standard Appointment (follow-up): $165 (for all existing patients regardless of treatment)

      3. Paid follow-ups recommended for New Patients, and existing patients who feel it’s necessary. These appointments reserve available time slots at patient’s convenience with what’s available.

      4. Children 17 and under are FREE **when accompanying a paying adult**

         (***Limited to 2 children / per adult / per visit***.)

WE WILL CONTACT YOU:

• Due to the volume of people requesting this, those that do not follow instructions will not be booked, and will be bumped to the end of the line when all requirements are fulfilled in the email. Please do NOT forget to follow all instructions.

This payment will be non-refundable, but transferrable (to another person known personally by you or to your next scheduled visit) if you:

1. Notify us in writing your need to cancel (7) days or more BEFORE your scheduled appointment

2. Notify us in writing with documentation and proof that you sustained an unavoidable emergency, and it will be left to our personal discretion on a case by case basis
